The Roadmap For New Patients

Interested in getting your medical cannabis card, but not sure if it’s right for you? Let The Giving Tree walk you through the process of obtaining your card to your first medical cannabis purchase.

Qualifying Conditions

First, do you qualify for a medical cannabis card? Medical conditions in Arizona include:
Cancer | Glaucoma | HIV | AIDS | Hepatitis C | ALS | Crohn’s Disease | Alzheimer’s Disease


Cachexia or wasting syndrome | Severe and chronic pain | Severe nausea

Seizures, including those characteristic of epilepsy | Severe or persistent muscle spasms

Getting Your Card

You’ll need to visit a licensed doctor and receive a referral. We recommend the following clinics:
Trichome Care Clinic, on Southern and Extension
Canna Releaf in Mesa, on Power and Brown
The Phoenix Certification Clinics, 6 locations in Greater Phoenix

Once you receive your referral, the next step is submitting your paperwork to the Arizona Department of Health Services. Each of our recommended clinics will assist you with this step.

Selecting Your Medicine

Each new patient is invited to come to our dispensary for a one-on-one consultation. We’ll provide you with information on methods of medication, how you can expect the medication to affect your body and mind, and answer any additional questions you have. You’ll also receive a free pre-roll just for stopping by! Our goal is to always make our patients feel welcome, from their first visit to their hundredth.

As of April 2016, more than 100,000 Arizonans have received their qualifying patient card. We invite you to take another step in your wellness and join them.
